That team is expanding, and we currently have exceptional opportunities to help our communities grow.The Patient Navigator reports to the Director of Cancer Services in consultation with the Chair of the Cancer Committee and is responsible for coordinating the care of patients with cancer (specify type as applicable). This position will assess the patient?s individual needs and develop a plan of care to meet those needs for physical, psychological, spiritual and social needs. The navigator will work with the oncology social worker and the Business Office financial counseling service to support oncology patients and their needs. This role includes providing education and support to patients, families, staff and caregivers. The goals of this position include improving patient care and flow, eliminating barriers, and improving the overall patient experience by connecting the patient and family with appropriate resources and services. This position will also work with Registry personnel to organize and assist with cancer conferences as needed.ASSESSMENT and DEVELOPMENT OF INDIVIDUALIZED PLAN OF CARE: Through initial and periodic assessments and through communication with patient/family, physicians and other staff, the patient navigator identifies physical, psychological, spiritual and social needs, and coordinates the development and implementation of plan of care to meet those needs effectively, efficiently, and at the appropriate level of care required.DOCUMENTATION IN EMR: Documents all patient and physician interactions in electronic medical record, including all education related encounters. Also utilizes other documentation tools as needed to ensure accurate and concise documentation (i.e. psychological assessment form); keeps referring physician informed of treatment goals and patient milestones. CARE COORDINATION: Coordinates the care of cancer patients, including completing an individualized needs assessment and care plan development for each patient to address physical, psychological, spiritual and social needs. a. Serves as patient advocate and resource to help prevent and/or resolve patient related clinical issues/conflict and reduce barriers to care. Refers to other services both within and outside of Mercy (i.e. financial, case management, palliative care, etc.) as needed in order to navigate the patient throughout the continuum. b. Serves as a facilitator for collaboration with multidisciplinary health team members.c. Focus on patient satisfaction including timely testing and treatments; actively seeks ways to improve patient flow.d. Ensures follow up with appropriate resources as identified (timely scheduling of tests, patient education, therapy, consults, etc.).4) PATIENT EDUCATION: Provides education to patients and families prior to initiating treatment that supplement and enhance the education that is given by the patient?s oncology physician or surgeon. Provision of individualized (to include language specific) site-specific educational tools that include written, video and/or face-to-face interactions with navigator. Also responsible to review patient educational materials annually and make recommendations for changes or additions.COMMUNICATION AND MULTIDISCIPLINARY MANAGEMENT: Works closely with physicians (radiation oncologists, medical oncologists, PCP, surgeons, etc) as primary patient liaison to support the physician-patient relationship while communicating with and educating as well as coordinating patient care in an effective and timely manner (i.e. assist with appointment scheduling, medication needs, and education on specific treatments/cancers as indicated). DATA COLLECTION: Assists with development of performance metrics to measure program success. Collects and analyzes outcome data, maintains dept statistics, records and resource files. Identifies areas of improvement and develops action plans accordingly. COMMUNITY EDUCATION and MARKETING OF SERVICES: a. Provides educational programs and in-services related to specific cancers to both hospital coworkers and community to increase awareness and knowledge related to diagnosis and treatment. b. Active and ongoing marketing plan directed at physicians and clinics to visit, educate and encourage appropriate referrals for patient navigation services in a timely fashion.CANCER/TUMOR CONFERENCE PARTICIPATION/MANAGEMENT: Work closely with physicians and Cancer Registry staff to collect patient data and identify speakers for cancer conference needs. SUPPORT GROUP FACILITATION: Facilitates support group(s) as indicated, following guidelines established by Mercy World Class Cancer Care recommendations.RESEARCH COORDINATION: Assists with research activities as indicated, following guidelines established by IRB and other regulatory agencies and disciplines.Qualifications:Because you?re committed to excellence, you understand the importance of being properly prepared for your role at Mercy.
