Position Summary
Performs case management activities to ensure the CDS Consumer’s needs are being met by the CDS Program. Utilizes monthly case management activities to monitor the Consumer’s ability to manage their attendant(s) in accordance with program guidelines, ensures the Consumer and their attendant meets the requirements of the CDS Program, connects the Consumer with applicable resources to assist the Consumer in managing their own care, ensures the Consumer’s needs are being met by the plan of care, provides training based on the Consumer’s needs, and assigns face-to-face referrals for home visits in the Consumer’s living environment as applicable. Responsible for case management documentation of program oversight. Responsible for the review and approval of payroll processing for the Consumer’s attendant.
Essential Duties and Responsibilities
• Responsible for reviewing and processing attendant timesheets and Electronic Visit Verification (EVV) corrections;
• Monitors and provides resolution on reports analyzing EVV and timesheet errors, holds and overtime;
• Provides ongoing training to Consumers based on their ability to supervise and self-direct their attendant;
• Monitors Consumers’ ability to self-direct their own care and the appropriateness and progress of the services being delivered;
• Identifies and submits appropriate face-to-face referrals to the CDS Coordinator team to complete home visits in the Consumer’s living environment;
• Performs monthly telephone case management activities with assigned Consumers in accordance with program guidelines;
• Documents monthly case management activities and every contact with Consumer and/or attendant adequately and addresses identified issues in a timely matter;
• Identifies and makes appropriate referrals to connect Consumer to resources based on their needs;
• Monitors attendant’s performance utilizing EVV and timesheets within the first 90-days of employment with the Consumer to ensure adequate understanding of the program and policies;
• Ensures Consumer needs are being met by the plan of care and coordinates care change needs with the Division of Health and Senior Services (DHSS);
• Participates in staff meetings and quality assessment reviews as required;
• Serves as backup to Care Support for Medicaid referrals by taking referral calls from potential Consumers, Clients, or family members, and following Integrity's referral process, as well as accurately submitting appropriate referral information to DHSS per DHSS' preferred method of referral submission;
• Complies with Compliance Plan;
• Complies with HIPAA Policies. Maintains confidentiality of information relating to the Consumer and family. Will discuss only those aspects necessary to the care and treatment of Consumer and family with those directly involved in the Consumer’s care;
• As business needs may change, may perform additional responsibilities assigned by supervisor that are reasonably related to the position.
Education and Experience Requirements
• High school diploma or equivalent required;
• Bachelor’s degree in Social Work, Human Services, or related field preferred.
• Minimum one (1) year experience working in the provision of care provided to the elderly, disabled, or infirm required;
• Minimum one (1) year previous experience in counseling or education, or administration, social services, or activity coordination for an elderly or group home setting preferred.
