Overview The Physical Therapist is responsible for evaluating and assessing needs of patients and formulating treatment plans. Provides therapy services defined in treatment plans, and works cooperatively with physicians, case managers, and staff. In addition, the Physical Therapist provides direction to Physical Therapy Assistants and Aides. Provides expert assessment by using the appropriate policy, protocols and standards of practice. By evaluating the appropriate documents, therapies, medications and patient responses to interventions. And by interpreting and initiating appropriate actions to changes in patient condition and monitoring data. Provides patient education by assessing and utilizing methods based on the patient and significant other's knowledge base, communication skills, developmental age, ability and readiness to learn. Appropriately manages the plan of care by initiating, developing, evaluating and revising the plan. Incorporates patient and significant others involvement to support the patient goal, as well as interdisciplinary plan of care. Reports pertinent information to staff and physicians and collaborates with the health-care team to plan/provide patient care as evidenced by documentation. Provides leadership by directing and coordinating assignments and care given by team members based on staff competencies, experience and scope of responsibility. Participates in the PI process and identifying opportunities to improve patient care delivery, including collecting data, monitoring and implementing changes. Enhances the learning opportunities for other staff, and by effectively contributing to the orientation of team members and students. Documentation is provided by ensuring that the patient record reflects the plan of care, patient care delivered and the status during his or her care and discharge. Ensures that the information is legible, concise, accurate, objective and timely, while meeting hospital standards Qualifications: Because you're committed to excellence, you understand the importance of being properly prepared for your role at Mercy. That's why you'll bring to your role the right set of qualifications: Education: Advanced graduate degree required from an accredited Physical Therapy program. Licensure: Physical Therapist Missouri license Experience: Previous successful Physical Therapy delivery is preferred. Certifications: BLS Basic Life Support Within 3 Months Of Hire Other: Excellent interpersonal, analytical and organizational skills are required.
